What is the CMS THA/TKA PRO-PM Measure?

Starting July 1, 2024, CMS made the THA/TKA PRO-PM mandatory for hospital inpatient procedures. This measure requires hospitals to report inpatient procedures and corresponding patient-reported outcome measures (PROMs) under the Hospital Inpatient Quality Reporting (IQR) program. The measure requires the collection of patient-reported outcomes both pre-operatively (within the 90 days prior to surgery) and one year post-operatively (between 300-425 days after surgery). The IQR program requires that 50% of patients must have complete data sets. Not meeting the data completion requirement results in reporting failure.

50% of patients must have both the preoperative and 1-year post-operative PROM data collected. APU is impacted by successfully meeting this capture rate.

The Risks Of Building A PRO Program Through An EHR

Navigating the world of PROMs can be a daunting task, especially when using an EHR as the mechanism for survey collection. Despite their versatility, EHRs are not typically purpose driven for low effort, high capture rate.

The measure also calculates the Substantial Clinical Benefit (SCB) for each patient, reflecting the change in functional PROM scores pre-operative to 1-year post-operative. For knee replacement patients, an improvement of 20 points in the Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS, JR) is expected, while hip replacement patients are expected to see an improvement of 22 points in the Hip Disability and Osteoarthritis Outcome Score for Joint Replacement (HOOS, JR). This culminates in a facility-level outcome score, also known as the Risk-Standardized Improvement Rate (RSIR), representing the percentage of patients who meet or exceed the SCB improvement threshold. A proportion of patients should meet or exceed an improvement threshold between their preoperative and post-operative PRO scores: 22 points for the HOOS JR and 20 points for the KOOS JR.

CMS calculates the RSIR by aggregating all patient-level results across the facility. For example, a 60% RSIR indicates that 60% of patients at this facility had a substantial improvement following their THA/TKA.

  • If your pre-operative capture rate is below 80% this is a signal that you have a high risk of failing the data completeness requirements.
  • If you cannot track your functional improvement by facility to ensure your average score meets benchmark requirements, your reputation is at risk.

  • Disruption to workflow: Whether collecting a survey while the patient is checking in for surgery or during a 1-year post-operative clinic visit, workflows need to be adjusted and extra time allocated for PROM collection.
  •  FTE resources are required: Both clinical and IT resources are necessary during implementation and on an ongoing basis for PROM collection and reporting. Expect to assign a full-time employee for every 100 procedures performed monthly.
  • Capture rate and facility-level outcome score reporting: Whether collecting on paper or electronically, leaders need ongoing visibility of the capture rate and improvement scores. EHRs do not have this reporting capacity natively
